Participation in Black River High School/Middle School Athletics requires that the following forms be completed and on file with the Athletic Director:
• Sports Permission Form
• Athletic Contract
• Extracurricular Activity Substance & Alcohol Abuse Abstention Agreement
• Release for Medical Treatment
• Pre-Participation Physical Evaluation Form
• A completed physical within the last two years. Beginning 2010 all physical will require the Well-exam Sports Participation Clearance Form
